The Waco ISD Employee Assistance Program, offered by American Fidelity is available to assist employees and their dependents who may be experiencing personal life problems, such as marital strife, legal or financial issues, stress, emotional problems, family problems, and drug- or alcohol- related problems. We encourage you to make use of Waco ISD EAP services whenever you feel it is necessary.
Dependents and partners residing in the employee’s household are fully covered. The EAP is available at no cost to the employee or family member and is completely confidential.

Form 1095-C

Electronic Form 1095-C
Employees have the ability to have their Form 1095-C delivered electronically through their Employee Service Center. This allows you immediate access to your form.
Please note that IRS due date for distributing 2022 forms is January 31, 2023.

Paper Form 1095-C
For employees who do not consent to electronic delivery of From 1095-C, a paper copy of the form will be mailed to the address on file. It is important to keep your mailing address updated in the system. If you have moved, please update your address in the Employee Service Center or complete an Address Change Form and send it to the Human Resources Department.

Additional Form 1095-C copies are available online through the Employee Service Center. If a replacement form is needed, please contact the Waco ISD Benefits Office at 254-755-9547.

What is a Form 1095-C?
- Form 1095-C provides information about the health coverage offered by Waco ISD and, in some cases, about whether you enrolled in this coverage.
- You may be required to submit information from Form 109-C on your federal tax return.
Who receives a Form 1095-C?
- Employees who enrolled in Waco ISD's group medical plan for one or more months in a calendar year will receive a Form 1095-C. Only one form is provided and includes all the individuals listed on your policy; you may need to make copies for your spouse or dependents.
What if a Form 1095-C is not available for me in the ESC?
- If you believe you met the criteria and did not receive a form, please contact the Waco ISD Benefits Office at 254-755-9547.
